MONTHLY PILATES UNLIMITED PROGRAM SERVICE AGREEMENT:
This is a rider to the membership agreement between Fitness Formula Clubs and member listed above. It is hereby understood that the member has requested to add the Pilates Unlimited Monthly Program membership dues* and will be billed an additional $219 monthly.

Services are billed with monthly membership dues and charged to your billing information on file. Current prorated month dues will be collected for your added Pilates Unlimited Monthly Program today. Program Registration is month to month and will automatically renew on the 16th of each month regardless of active participation in the classes.

Monthly program dues are non-refundable and lack of use does not constitute cancellation. There is a 3 month minimum commitment and I understand that I will be billed for a minimum of 3 months.

I understand all Pilates Unlimited Monthly Program Sessions expire on the last day of the month.

I understand that I may cancel a Pilates Unlimited Monthly Program Class with 12 hours notice directly to the instructor via FFC e-mail or through the FFC+ app. I also understand that I will be charged for my full session fee if I do not provide 12 hour cancellation notice for a Pilates Unlimited Monthly Program Class. Classes not attended or cancelled within the 12 hours notice period will be subject to a late cancel/no show fee of $25.

Fees associated with the Pilates Unlimited Monthly Program Sessions are non-refundable, but transferable to other club services excluding any membership dues or payments. I understand that if I choose to break this agreement, any completed sessions will be adjusted to reflect the single session price. This total will be subtracted to determine transferable funds.

Pilates Unlimited Monthly Program EFT billing cancellation requests must be submitted in writing by the 15th to impact the next billing cycle. Cancellation submissions after the 15th will incur one additional billing cycle.

Your final month is the month of your last billed service payment. All services will be terminated at the end of your final month.

*Class schedule and class size restrictions may apply, Some classes will require pre-registration or RSVP to ensure a spot.
